General Warnings
  • Pulmonary Toxicity

    This medicine may cause severe lung injury which includes hypersensitivity pneumonitis and Interstitial pneumonitis. Discontinue the treatment as soon as these symptoms appear until the exact cause is identified.

  • Worsened Arrhythmia

    This medicine may cause exacerbation of the arrhythmias. This effect is more common in the patients being treated with other antiarrhythmic medicines or any changes in the dose. Close monitoring of potassium, magnesium levels is necessary before initiation of the treatment. Any symptoms of heart palpitations should be reported to the doctor.

  • Thyrotoxicosis

    This medicine may increase the thyroid levels and may lead to a serious condition called thyrotoxicosis. Close monitoring of thyroid levels is necessary. Any symptoms of irritability, irregular heartbeats, nervousness and weight loss should be reported to the doctor.

  • Impaired liver function

    This medicine may increase liver enzymes levels. Any signs of elevated liver enzymes should be reported to the doctor. Reduce the dose or discontinue the medicine if the value increases by 3 times the normal.

How to use Cordarone tablet 200 mg 3×10’s

IV inf over a period of 20 min to 2 hour. Diluted in 250 mL in 5% DW; may be followed by repeated infusions up to 1200 mg i.e. (approximately 15 mg/kg body wt) in up to 500 mL 5% DW per 24 hour, the rate of infusion based on clinical judgement. May be given by slow IV inj of 150-300 mg in 10-20 mL of 5% DW over 1-2 min.
Compatibility: Stable in D5W, KCl.
Y-site administration:
Drug to Drug IV Compatibility: Amikacin, bumetanide, calcium gluconate, ceftriaxone, ciprofloxacin, diltiazem, dobutamine, dopamine, epinephrine, esmolol, famotidine, fentanyl, fluconazole, gentamicin, insulin regular, labetalol, linezolid, lorazepam, methylprednisolone, midazolam, milrinone, morphine, nitroglycerin, norepinephrine, phenylephrine, tobramycin, vancomycin, vasopressin, vecuronium.
Drug to Drug IV Incompatibility: Aminophylline, cefamandole, digoxin, heparin, imipenem, levofloxacin, piperacillin/tazobactam, sodium bicarbonate.
In syringe: Incompatible with heparin.
When admixed: Incompatible with floxacillin.
Stability: Store at controlled room temperature, 20°C-25°C (68°F-77°F). Excursions are permitted to 15°C-30°C (59°F-86°F). Protect from light. Dispense in a light-resistant, tight container, keep tightly closed.

UNLICENSED USE
Children: Supraventricular and ventricular arrhythmias: ORAL: Neonate, initially 5-10 mg/kg twice daily for 7-10 days, then reduced to maintenance dose of 5-10 mg/kg once daily; 1 mth-3 yr, initially 5-10 mg/kg (max 200 mg) twice daily for 7-10 days, then reduced to maintenance dose of 5-10 mg/kg once daily (max 200 mg daily). IV INFUSION: Neonate, initially 5 mg/kg over 30 minutes then 5 mg/kg over 30 minutes every 12-24 hr; 1 mth-3 yr, initially 5-10 mg/kg over 20 minutes-2 hr then by continuous infusion 300 mcg/kg/hr, increased according to response to max. 1.5 mg/kg/hr; do not exceed 1.2 gm/24 hr.
Ventricular fibrillation or pulseless ventricular tachycardia refractory to defibrillation: IV INJECTION: Neonate, 5 mg/kg over at least 3 minutes; 1 mth-3 yr, 5 mg/kg (max 300 mg) over at least 3 minutes.

what are the adverse effects of Cordarone tablet 200 mg 3×10’s?

Severe bradycardia, headaches, fatigue, nightmares, hallucinations & depression (dose-related), nausea, vomiting, abdominal pain & constipation, corneal deposits, skin changes, ataxia, tremors, pulmonary changes, arthropathies, marked body weight loss. Hypothyroidism, hyperthyroidism, corneal microdeposits, raised serum transaminases, acute liver disorders, extrapyramidal tremor, sleep disorders, photosensitivity, optic neuropathy and neuritis, Stevens Johnson syndrome or toxic epidermal necrolysis.

What happens if I miss a dose of Cordarone tablet 200 mg 3×10’s?

Adults: Pulseless ventricular fibrillation or ventricular tachycardia: IV push or IO Initial 300 mg rapid bolus, if pulseless VT or VF continues after subsequent defibrillation or recurs, give supplemental dose of 150 mg.
Ventricular arrhythmias: Oral. 800-1600 mg/day in 2-3 divided doses for 1-3 wk, then when adequate control achieved, reduce to 600-800 mg/day in 1-2 divided doses for 1 mth; maintenance 400 mg/day. Lower doses recommended for supraventricular arrhythmias.
Children: Under 3 yr, not recommended. Supraventricular and ventricular arrhythmias: 3-12 yr, initially 5-10 mg/kg (max 200 mg) twice daily for 7-10 days, then reduced to maintenance dose of 5-10 mg/kg once daily (max 200 mg daily); 12-18 yr, 200 mg 3 times daily for 1 wk then 200 mg twice daily for 1 wk then usually 200mg daily adjusted according to response. IV infusion: 3-18 yr, initially 5-10 mg/kg over 20 minutes-2 hr then by continuous infusion 300 mcg/kg/hr, increased according to response to max. 1.5 mg/kg/hr; do not exceed 1.2 gm/24 hr.
Ventricular fibrillation or pulseless ventricular tachycardia refractory to defibrillation: IV injection. 3-18 yr, 5 mg/kg (max 300 mg) over at least 3 minutes.
