Allergy
This medicine is not recommended for use if you have a history of allergy to Epotin Injection 4000 IU 6 Vial or albumin or any other ingredient present in the dosage form.

Non-chemotherapy associated anemia in cancer patients
This medicine is not recommended for use to treat anemia in cancer patients who are are not receiving any chemotherapy medication.

High blood pressure
This medicine is not recommended for use in patients who have high blood pressure and are not taking any medicine to control it.

Pure red cell aplasia
This medicine is not recommended for use if the patient has had a history of pure red cell aplasia after using this medicine or other medicines belonging to the Eryhtropoietin class of drugs.

Use of multi-dose vials
Use of multi-dose vials of this medicine is not recommended for children or pregnant women. The risk of severe adverse effects are increasingly high in such cases.

Not for immediate effect
This medicine should not be used in cases where an immediate increase in red blood cell count is desired.

This medicine is not recommended for use in patients who are being treated with biological or radiological medications. It is also not recommended for use in patients who are receiving myelosuppressive therapy for a curable form of cancer.

How to use Epotin Injection 4000 IU 6 Vial
IV: Patients with CRF on dialysis: May be administered IV Bolus into the venous line after dialysis. Patients with CRF not on dialysis: May be administered IV or SC. Single-dose 1 mL vial contains no preservative. Use one dose per vial. Do not re-enter vial. Discard unused portions. Multidose 1 mL or 2 vial contains preservative. For minimal dilution, mix with bacteriostatic 0.9% sodium chloride, containing 20 mL of 0.9% sodium chloride and benzyl alcohol as the bacteriostatic agent. Dilutions of 1:10 and 1:20 (1 part epoetin: 19 parts sodium chloride) are stable for 18 hours at room temperature. Results showed no loss of epoetin alfa after a 1:20 dilution. 250 mcg/ mL albumin remaining after a 1:10 dilution of formulated epoetin alfa should be sufficient to prevent it from binding to commonly encountered containers. Prior to SC administration, preservative free solutions may be mixed with bacteriostatic NS containing benzyl alcohol 0.9% in a 1:1 ratio. Do not shake vial while preparing to avoid denaturing glycoprotein, inactivating drug.
Compatibility: Stable in D10W with albumin 0.05%, D10W with albumin 0.1%. Incompatible with D10W with albumin 0.01%, D10W, NS.
Stability: Vials should be stored at 2°C-8°C (36°F-46°F). Do not freeze or shake. Vials are stable for 2 weeks at room temperature. Single-dose 1 mL vial contains no preservatives. Use one dose per vial. Do not re-enter vial. Discard unused portions. Multidose 1 mL or 2 mL vial contains preservative. Store at 2°C-8°C after initial entry and between doses. Discard 21 days after initial entry.
what are the adverse effects of Epotin Injection 4000 IU 6 Vial?
Raised BP, headache, thrombosis, flu-like symptoms, seizures, skin reactions, rash, stroke, GI upset, palpebral oedema, musculoskeletal pain, cough, pyrexia. Rarely, pure red cell aplasia; withdraw and do not transfer to other erythropoietins. Inj site reactions.
What happens if I miss a dose of Epotin Injection 4000 IU 6 Vial?
Haemodialysis: Adults: Initially 50 IU/kg three times per wk by SC or IV inj following dialysis. Increase if necessary by 25 IU/kg three times per wk every 4 wk to achieve a Hb 10-12 gm/dl. Maintenance, 75-300 IU/kg per wk. Children: Initially 50 IU/kg three times per wk by IV inj following dialysis. Increase if necessary by 25 IU/kg three times per wk every 4 wk to achieve a Hb 9.5-11 gm/dl. Maintenance, under 10 kg, 75-150 IU/kg; 10-30 kg, 60-150 IU/kg; over 30 kg, 30-100 IU/kg. All three times per wk.
Peritoneal dialysis: Adults: 50 IU/kg twice wkly by IV inj increase if necessary by 25 IU/kg twice wkly every 4 wk to achieve a Hb 10-12 gm/dl. Maintenance, 25-50 IU/kg per wk in two divided doses.
Non-dialysis: Adults: Initially 50 IU/kg three times per wk by IV inj. Increase if necessary by 25 IU/kg three times per wk every 4 wk to achieve a Hb 10-12 gm/dl. Maintenance, 17-33 IU/kg per wk in three divided doses.
