What is it prescribed for?

  • Prophylaxis of ischemic complications in unstable angina

    This medicine is used to prevent the blockage of coronary arteries (major blood vessels carrying blood and oxygen to the heart) and associated complications. A blockage of the coronary artery may result in a heart attack, anginal pain, and heart rhythm abnormalities. This medicine is administered along with aspirin to prevent these complications.

  • Prophylaxis for Deep Vein Thrombosis

    This medicine is used for the prevention of blood clot formation in a deep vein, most commonly in the legs.

  • Deep Vein Thrombosis Prophylaxis after surgery

    This medicine is used for the prevention of blood clot formation during and after major surgical procedures such as hip replacement surgery, major abdominal surgeries, etc.

General Warnings
  • Hemorrhage and risk factors

    Use of this medicine should be initiated only after internal bleeding or hemorrhage and associated risk factors are ruled out. All the underlying causes of bleeding such as peptic ulcer disease, hemophilia, impaired hemostasis, etc. should also be ruled out before the administration of this medicine.

  • Thrombocytopenia

    Use of this medicine may cause low levels of platelets (thrombocytopenia). It is advised to monitor the blood platelet levels at regular intervals while this medicine is administered. Appropriate dose adjustments or replacement with a suitable alternative may be necessary based on the clinical condition of the patient.

  • Benzyl alcohol as preservative

    Dosage forms containing benzyl alcohol as a preservative should not be administered to neonates, infants, pregnant or breastfeeding women. Benzyl alcohol may cause a fatal adverse event known as Gasping syndrome in neonates and infants.

  • Liver/kidney disease

    This medicine should be used with caution in patients having an impairment of normal liver/kidney function. Appropriate dose adjustments and frequent clinical monitoring are recommended for such patients. Replacement with a suitable alternative may be necessary based on the clinical condition.

General Instructions

Administer Daltehep 5000 IU Injection exactly as instructed by the doctor. It should not be injected into the muscles. It is advised to administer this medicine under the supervision of a doctor in a clinical setup with regular monitoring of blood clotting time and other related parameters. How to use Fragmin Injection 10000 IU 1 Pre filled Syringex10 mL

Administer deep SC only. May be injected in a U-shape to the area surrounding the navel, the upper outer side of the thigh, or the upper outer quadrangle of the buttock. Vary injection site daily. Use thumb and forefinger to lift a fold of skin when injecting dalteparin to the navel area or thigh. Insert needle at a 45 to 90 degree angle the entire length of needle should be inserted. Apply pressure to injection site. Do not massage.
Compatibility: Dalteparin injection should not be mixed with other injections or infusions unless specific compatibility data are available that support such mixing.
Stability: Store at 20°C-25°C (68°F-77°F). After first penetration of the rubber stopper, store the multi dose vials at room temperature for upto two weeks. Discard any unused solution after two weeks.

UNLICENSED USE
Children: Treatment of thrombotic episodes: SC injection. Neonate, 100 units/kg twice daily; 1 mth-12 yr, 100 units/kg twice daily; 12-18 yr, 200 units/kg (max 18000 units) once daily, if increased risk of bleeding reduced to 100 units/kg twice daily.
Treatment of venous thromboembolism in pregnancy: SC injection. 2-18 yr, early pregnancy body-weight under 50 kg, 5000 units twice daily; body-weight 50-70 kg, 6000 units twice daily; body-weight 70-90 kg, 8000 units twice daily; body-weight over 90 kg, 10000 units twice daily.
Prophylaxis of thrombotic episodes: SC injection. Neonate, 100 units/kg once daily; 1 mth-12 yr, 100 units/kg once daily; 12-18 yr, 2500-5000 units once daily.

Bleeding at high doses. Heparin like side effects, transient raised liver enzyme, thrombocytopenia, haematoma at inj site.

What happens if I miss a dose of Fragmin Injection 10000 IU 1 Pre filled Syringex10 mL?

Adults: General surgery: 2500 IU by SC inj into abdominal wall or lateral part of thigh 1 to 2 hr pre-op., then 2500 IU daily until fully mobile (generally 5-7 days or longer).
High risk surgery: 2500 IU by SC inj into abdominal wall or lateral part of thigh 1 to 2 hr pre-op., repeated 8-12 hr later; or 5000 IU by SC Inj on the evening pre-op. Then 5000 IU daily until fully mobile (generally 5-7 days or longer).
Hip surgery: 5000 IU by SC Inj on the evening prior to surgery, then 5000 IU each evening for next 5 wk.
Haemodialysis/haemofiltration: If no additional bleeding risk, 30-40 IU/kg by IV bolus then 10-15 IU/kg/hr by IV infusion. Alternatively if procedure less than 4 hr, as above or 5000 IU by IV bolus. If high bleeding risk, 5-10 IU/kg by IV bolus then 4-5 IU/kg/hr by IV infusion.
DVT and PE: Adults: If no additional bleeding risk, 200 IU/kg by SC inj as a single daily dose; max 18000 IU daily. If high bleeding risk, 100 IU/kg by SC Inj twice daily. Give anticoagulants concurrently, use combined treatment for minimum 5 days.
Unstable coronary artery disease: 120 IU/kg SC Inj every 12 hr for 5-8 days; max 10000 IU/12 hr.
Children: Not recommended.
