Major surgery

An alternate medicine should be considered based on the clinical condition.

How to use Labetalol Injection IV 50 mg 1 Vialx10 mL
Administer slowly over 2 min by direct IV or by continuous IV infusion, at an initial rate of 2 mg/min with subsequent adjustments based on BP response. Standard diluent: 500 mg/250 mL D5W. Keep patient in supine position for 3 hours after IV administration.
Compatibility: Stable in D5LR, D5NS, D5W, LR, NS.
Y-site administration:
Drug to Drug IV Compatibility: Amikacin, amiodarone, aztreonam, bumetanide, calcium gluconate, cimetidine, diltiazem, diphenhydramine, dobutamine, dopamine, enalapril, epinephrine, esmolol, famotidine, fentanyl, fluconazole, gentamicin, hydromorphone, imipenem, levofloxacin, linezolid, lorazepam, magnesium sulphate, methylprednisolone, metoclopramide, metronidazole, midazolam, milrinone, nitroglycerin, nitroprusside, norepinephrine, ondansetron, phenylephrine, propofol, ranitidine, tobramycin, vancomycin, vasopressin, vecuronium.
Drug to Drug IV Incompatibility: Acyclovir, amphotericin B cholesteryl sulphate complex, cefoperazone, ceftriaxone, dexamethasone, hydrocortisone, insulin regular, lansoprazole, nafcillin, pantoprazole, piperacillin/tazobactam, thiopental, warfarin.
When admixed: Incompatible with sodium bicarbonate.
Stability: Store at room temperature. Do not freeze. Protect from light. The solution is clear to slightly yellow. For parenteral admixture, stability at room temperature is 25°C and refrigeration temperature (4°C) for 3 days.
UNLICENSED USE
Children: Hypertensive emergencies: INJECTION: IV infusion: Neonate, 500 mcg/kg/hr adjusted at intervals of at least 15 minutes according to response, max 4 mg/kg/hr; 1 mth-12 yr, initially 0.5-1 mg/kg/hr adjusted at intervals of at least 15 minutes according to response, max. 3 mg/kg/hr; 12-18 yr, 30-120 mg/hr adjusted at intervals of at least 15 minutes according to response. In hypertensive encephalopathy reduce blood pressure to normotensive level over 24-48 hr (more rapid reduction may lead to cerebral infarction, blindness and death). If child fitting, reduce blood pressure rapidly, but not to normal levels.
Hypertension: ORAL: 1 mth-12 yr, 1-2 mg/kg 3-4 times a day; 12-18 yr, initially 50-100 mg twice daily increased if required at intervals of 3-14 days to usual dose of 200-400 mg twice daily (3-4 divided doses if higher), max 2.4 gm daily. INJECTION: IV injection: 1 mth-12 yr, 250-500 mcg/kg as a single dose; max. 20 mg; 12-18 yr, 50 mg over at least 1 minute, repeated after 5 minutes if necessary; max. total dose 200 mg.
what are the adverse effects of Labetalol Injection IV 50 mg 1 Vialx10 mL?
Cardiovascular effects including bradycardia, postural hypotension. Rarely, heart failure, increased AV block, cardiac arrhythmias. Raynaud’s phenomenon cold extremities, sexual dysfunction. GI, sleep and CNS disturbances. Dyspnoea rarely, bronchospasm. Skin reactions, withdraw if unexplained dry eyes or rash. Fatigue, paraesthesia, peripheral neuropathy, arthralgia, myopathy, altered blood sugar, haematology and lipid levels.
What happens if I miss a dose of Labetalol Injection IV 50 mg 1 Vialx10 mL?
ORAL: Adults: Initially 100 mg twice daily with food increasing if necessary at 14 day intervals; max 2.4 gm daily in three or four divided doses. Elderly: Initially 50 mg twice daily. Children: Not recommended.
INJECTION: Adults: IV: 20 mg (0.25 mg/kg for 80 kg patient) by IV injection over 2 min; may be administered 40-80 mg at 10 min intervals; up to 300 mg total dose. IV Infusion: Initial 2 mg/min; titrate to response up to 300 mg total dose, if needed. Children: Not recommended.
