General Warnings
  • Liver injury

    This medicine is not recommended in the patients with the liver injury. The concentration of this medicine may increase in these patients. The baseline of the liver function tests should be obtained before starting the treatment.

  • Kidney injury

    This medicine is not recommended in the patients with the kidney injury. It may cause kidney injury at higher doses. The dose of this medicine should be adjusted based on the CrCl.

  • Lung toxicity

    This medicine may cause severe lung injuries like pulmonary fibrosis, pulmonary interstitial infiltrates. Any symptoms of breathing difficulty and signs of lung impairment should be informed to the doctor. The treatment should be discontinued if any symptoms appear.

  • Blood cell count

    This medicine may lower the white blood cell count (neutrophils) and platelets. Close monitoring of blood cell count is necessary. The baseline of the blood cell count should be obtained before initiation of the treatment. Do not start the treatment if the counts are less.

  • Skin reactions

    The medicine may cause severe skin reactions stevens-johnson syndrome, skin necrosis, and skin ulceration. Inform the doctor if any symptoms appear. The treatment should be discontinued.

  • Gastrointestinal injury

    This medicine is not recommended for use in patients with gastrointestinal disorders like diarrhea and ulcerative stomatitis. Inform the doctor if you have any gastrointestinal disorders before initiation of the therapy.

  • Infections

    The chances of occurring infections are more in the patients receiving this medicine. Hold the chemotherapy and start with the broad-spectrum antibiotics if there are any signs and symptoms of fever. Close monitoring of the body temperature and the blood cell count is necessary.

How to use Methotrexate Injection 50 mg 1 Vialx2 mL

It may be administered IM, IV or IT. IV administration may be slow push, short bolus inf or 24-42 hour continuous inf. Dilute powder with D5W or NS to a conc. of = 25 mg/mL (20 mg and 50 mg vials)and 50 mg/mL (1 gm vial). Intrathecal solutions may be reconstituted to 2.5 to 5 mg/mL with NS, D5W, LR or Elliott’s B solution. Use preservative free preparations for IT or high dose administration.
Stability: Store at 20°C-25°C (68°F-77°F); excursions are permitted to 15°C -30°C (59°F-86°F). Protect from light.

What happens if I miss a dose of Methotrexate Injection 50 mg 1 Vialx2 mL?

Adults: Antineoplastic: Burkitt’s Lymphoma: Stages I-II: Orally: 10-25 mg/day for 4-8 day for several courses; separate by 7-10 day rest period.
Lymphosarcomas: Stage III: Orally: 0.625-2.5 mg/kg/day.
Leukaemia: Induction: 3.3 mg/m² with prednisone every day. Remission Maintenance: 15 mg/m² Orally/IM twice wkly or 2.5 mg/kg IV every 14 day.
Mycosis fungoides: 5-50 mg every wk. If poor response, give 15-37.5 mg twice wkly.
Osteosarcoma: Initial: 12 gm/m² IV, increase to 15 gm/m² if peak levels of 1000 micro-molar not reached at end of infusion.
Choriocarcinoma/trophoblastic diseases: Orally/IM: 15-30 mg every day x 5 day, rest gt;1 wk & repeat 3-5 times.
Rheumatoid arthritis: Initial: 7.5 mg once wkly or 2.5 mg every 12 hr x 3 doses once wkly. Max: 20 mg/wk. Reduce to lowest effective dose.
Psoriasis: Usual: 10-25 mg/wk Orally/IV/IM until adequate response achieved or use 2.5 mg every 12 hr x 3 doses. Titrate: May increase gradually. Max: 30 mg/wk.
Children: Meningeal leukaemias: Intrathecal: Dilute preservative free Methotrexate to 1 mg/mL. Dose every 2-5 day. gt;3 yr: 12 mg. 2 yr: 10 mg. 1 yr: 8 mg. lt;1 yr: 6 mg.
Juvenile rheumatoid arthritis: Children 2-16 yr: 10 mg/m² once wkly. Adjust dose to optimal response.
