How to use Noctrum Injection 5 mg 5 Ampx5 mL

IM: Give deep IM into large muscle. IV: Administer by slow IV injection over at least 2-5 min at a concentration of 1-5 mg/mL or by IV infusion. Continuous infusions should be administered via an infusion pump.
Compatibility: Stable in D5NS, D5W, NS. Incompatible with LR.

UNLICENSED USE
Adults: Status epilepticus refractory to standard therapy: Intubation required; adjust dose based on hemodynamics, seizure activity and EEG. IV: 0.15-0.3 mg/kg (usual dose: 5-15 mg); may repeat every 10-15 minutes as needed.
Status epilepticus, prehospital treatment: Administered by paramedics when convulsions last gt;5 minutes or if convulsions are occurring after having intermittent seizures without regaining consciousness for gt; 5 minutes. IM: 10 mg once.
Children: Status epilepticus, febrile convulsions: IV: Neonate, initially by IV injection 150-200 mcg/kg followed by continuous IV infusion of 60 mcg/kg/hr (increased by 60 mcg/kg/hr every 15 minutes until seizure controlled), max. 300 mcg/kg/hr; 1 mth-18 yr, initially by IV injection 150-200 mcg/kg followed by continuous IV infusion of 60 mcg/kg/hr (increased by 60 mcg/kg/hr every 15 minutes until seizure controlled), max 300 mcg/kg/hr.

Confusion, euphoria, hallucinations, headache, dizziness, ataxia, anterograde amnesia, paradoxical reactions, convulsions, GI upset, cardiorespiratory depression, hypotension, rash, slight tachycardia, vasodilation, dyspnoea, erythema, thrombophlebitis, thrombosis, pain at injection site. Paradoxical reactions, anterograde amnesia. Tolerance and dependence may develop during long-term sedation. Judgement and dexterity may be impaired; warn patient.

What happens if I miss a dose of Noctrum Injection 5 mg 5 Ampx5 mL?

Adults: The dose needs to be individualized based on the patient’s age, underlying diseases and concurrent medications. Consider reducing dose by 20% to 50% in elderly, chronically ill, or debilitated patients and those receiving opioids or other CNS depressants.
Preoperative/preprocedural sedation: Healthy adults lt; 60 yr: IM: 0.07-0.08 mg/kg 30-60 minutes prior to surgery/procedure, usual dose 5 mg. IV: 0.02-0.04 mg/kg, repeat every 5 minutes as needed to desired effect or up to 0.1-0.2 mg/kg.
Conscious sedation: IV: Healthy adults lt; 60 yr: Initial 1 mg; no more than 2.5 mg over a period of 2 minutes. Additional doses after 2 minutes if necessary. Total dose above 5 mg is generally not needed. Maintenance 25% of dose used to reach sedative effect. Above 60 yr age, consider dose reduction as for elderly as mentioned below.
Anaesthesia: IV: Induction: Adults lt; 55 yr: Patients without premedication: 0.3-0.35 mg/kg over 20-30 sec; after 2 minutes, may repeat if necessary at 25% of initial dose every 2 minutes, up to a total dose of 0.6 mg/kg in resistant cases. Premedicated patients: Usual dosage range: 0.05-0.2 mg/kg, usually 0.2 mg/kg over 5-10 sec. When used with other anaesthetic drugs (co-induction), the dose is lt;0.1 mg/kg. Maintenance: 0.05 mg/kg as needed, or continuous infusion 0.015-0.06 mg/kg/hr (0.25-1 meg/kg/minute).
Sedation in mechanically-ventilated patients: IV: Initial dose: 0.01-0.05 mg/kg (0.5-4 mg); may repeat at 5 to 15 minutes intervals until adequate sedation; maintenance infusion, 0.02-0.1 mg/kg/hr (0.3-1.7 meg/kg/minute). Titrate to reach desired level of sedation. Elderly: Anaesthesia: IV: Induction: Adults gt;55 yr: Patients without premedication: Initial dose: 0.3 mg/kg. Premedicated patients: Reduce dose by at least 20%.
Elderly: Conscious sedation: IV: Initial: 0.5 mg slow IV; give no more than 1.5 mg in a 2 minute period; if additional titration is needed, give no more than 1 mg over 2 minutes, waiting another 2 or more minutes to evaluate sedative effect, a total dose of gt;3.5 mg is rarely necessary.
Elderly: Preoperative/preprocedural sedation: gt;60 yr (without concomitant opioid administration): IM: 2-3 mg (or 0.02-0.05 mg/kg) 30-60 minutes prior to surgery/procedure; some may only require 1 mg (or 0.01 mg/kg) if anticipated intensity and duration of sedation is less critical.
Children: Not recommended for children under 6 mth for premedication and conscious sedation; not recommended for oral use or by buccal administration for conscious sedation. Oromucosal solution not recommended for children under 3 mth; injection not recommended for status epilepticus or febrile convulsions.
Conscious sedation for procedures: ORAL: 1 mth-18 yr, 500 mcg/kg, max 20 mg, 30-60 minutes before procedure. By buccal administration: 6 mth-10 yr, 200-300 mcg/kg, max 5 mg; 10-18 yr, 6-7 mg, max 8 mg if 70 kg or over. By RECTUM: 6 mth-12 yr, 300-500 mcg/kg 15-30 minutes before procedure. IV INJECTION over 2-3 minutes 5-10 minutes before procedure: 1 mth-6 yr, initially 25-50 mcg/kg, increased if necessary in small steps, max total dose 6 mg; 6-12 yr, initially 25-50 mcg/kg, increased if necessary in small steps, max total dose 10 mg; 12-18 yr, initially 25-50 mcg/kg, increased if necessary in small steps, max total dose 7.5 mg.
Premedication: ORAL: 1 mth-18 yr, 500 mcg/kg, max 20 mg, 15-30 minutes before the procedure. By RECTUM: 6 mth-12 yr, 300-500 mcg/kg 15-30 minutes before induction.
Induction of anaesthesia (but rarely used): IV INJECTION: 7-18 yr, initially 150 mcg/kg, max 7.5 mg, given in steps of 50 mcg/kg, max 2.5 mg, over 2-5 minutes; wait for 2-5 minutes then give additional doses of 50 mcg/kg, max 2.5 mg, every 2 minutes if necessary, max total dose 500 mcg/kg (not exceeding 25 mg).
Sedation in intensive care: IV injection and continuous IV infusion: Neonate less than 32 wk corrected gestational age, 60 mcg/kg/hr by continuous IV infusion, reduced after 24 hr to 30 mcg/kg/hr; adjusted according to response, max treatment duration 4 days; Neonate over 32 wk corrected gestational age, 60 mcg/kg/hr by continuous IV infusion adjusted according to response, max treatment duration 4 days; 1-6 mth, 60 mcg/kg/hr by continuous IV infusion adjusted according to response; 6 mth-12 yr, initially 50-200 mcg/kg by slow IV injection over at least 3 minutes followed by 30-120 mcg/kg/hr by continuous IV infusion adjusted according to response; 12-18 yr, initially 30-300 mcg/kg by slow IV injection given in steps of 1-2.5 mg every 2 minutes followed by 30-200 mcg/kg/hr by continuous IV infusion adjusted according to response.
Status epilepticus, febrile convulsions: By buccal administration, the following dose repeated once after 10 minutes if necessary: Neonate, 300 mcg/kg; 1-3 mth, 300 mcg/kg, max 2.5 mg; 3 mth-1 yr, 2.5 mg; 1-5 yr, 5 mg; 5-10 yr, 7.5 mg; 10-18 yr, 10 mg.
