How to use Tazomax Injection 4.5 gm 1 Vial

IV: Administer by IV infusion over 30 min.
Reconstitution: Use single-dose vials immediately after reconstitution (discard unused portions after 24 hours at room temperature and 48 hours if refrigerated). Reconstitute with 5 mL of diluent per 1 gm of piperacillin and then further dilute. Compatible diluent include NS, SWFI, dextran 6%, D5W, D5W with potassium chloride 40 mEq, bacteriostatic saline and water.
Compatibility: Stable in dextran 6% in NS, D5W, NS, SWFI, bacteriostatic water. Incompatible with LR.

Drug to Drug IV Compatibility: Amikacin, aztreonam, bumetanide, calcium gluconate, cefepime, cimetidine, cisatracurium, dexamethasone, diphenhydramine, dopamine, enalapril, epinephrine, esmolol, fentanyl, fluconazole, furosemide, heparin, hydrocortisone, hydromorphone, lansoprazole, linezolid, lorazepam, magnesium sulphate, methylprednisolone, metoclopramide, metronidazole, milrinone, morphine, nitroglycerin, nitroprusside, norepinephrine, ondansetron, phenylephrine, ranitidine, vasopressin.
Drug to Drug IV Incompatibility: Acyclovir, alatrofloxacin, amiodarone, amphotericin B, amphotericin B cholesteryl sulphate complex, azithromycin, chlorpromazine, ciprofloxacin, cisplatin, dacarbazine, daunorubicin, diltiazem, dobutamine, doxorubicin, doxorubicin liposome, doxycycline, droperidol, famotidine, ganciclovir, gatifloxacin, gemcitabine, haloperidol, hydroxyzine, idarubicin, insulin regular, labetalol, levofloxacin, midazolam, minocycline, mitomycin, mitoxantrone, nalbuphine, prochlorperazine edisylate, promethazine, streptozocin, tobramycin, vancomycin.
Stability: Store at controlled room temperature. After reconstitution, solution is stable in NS or D5W for 24 hours at room temperature and 7 days when refrigerated.

Skin reactions, convulsions, Gl disturbances, allergic reactions, superinfection, hypokalaemia. Rarely leucopenia, interstitial nephritis, renal failure, haemolytic anaemia, hepatitis, cholestatic jaundice, pseudomembranous colitis (discontinue immediately). Positive Coombs test.

What happens if I miss a dose of Tazomax Injection 4.5 gm 1 Vial?

Adults: 4.5 gm by slow IV injection or infusion eight hourly.
Neutropenic adults: 4.5 gm by slow IV injection or infusion six hourly with aminoglycosides.
Children: Neutropenic children: 90 mg/kg by slow IV injection or infusion six hourly with aminoglycosides; max 4.5 gm six hourly.
Appendicitis: Adults: Under 2 yr, not recommended. 2-12 yr: 112.5 mg/kg by slow IV injection or infusion eight hourly; max 4.5 gm eight hourly.
