Allergy
Not recommended in the patients allergic to Vancolone Injection 1 gm 1 Vial .

Hearing problem
Excessive intravenous doses of this medicine may alter the hearing ability. Any symptoms of the hearing problem should be reported to the doctor.

Impaired kidney function
This medicine may increase the risk of kidney injury, especially in the elderly population. The dose of this medicine should be adjusted based on the CrCl. Any signs and symptoms of kidney injury like fluid accumulation, weight gain should be reported to the doctor.

Drug-resistance
Usage of Vancolone Injection 1 gm 1 Vial without sufficient proof or suspicion of a bacterial infection should be avoided. Irrational dosing might fail in providing the benefits and even cause toxicity. It may also increase the risk of development of bacteria that is drug resistant.

How to use Vancolone Injection 1 gm 1 Vial
IV: Administer vancomycin by IV intermittent infusion over at least 60 min at a final conc not to exceed 5 mg/ mL. Administration of antihistamines just before the infusion may also prevent or minimize Red man syndrome.
Compatibility: Stable in dextran 6% in NS, D5W, D5% NS, RL, D5% RL, NS, D10W.
Y-site administration:
Drug to Drug IV Compatibility: Acyclovir, amikacin, amiodarone, bumetanide, calcium gluconate, cimetidine, cisatracurium, diltiazem, diphenhydramine, dobutamine, dopamine, enalapril, epinephrine, esmolol, famotidine, fentanyl, fluconazole, gentamicin, hydromorphone, imipenem, insulin regular, labetalol, levofloxacin, linezolid, lorazepam, magnesium sulphate, meropenem, methylprednisolone, metoclopramide, midazolam, milrinone, morphine, nitroglycerin, nitroprusside, norepinephrine, ondansetron, phenylephrine, ranitidine, tobramycin, vasopressin, vecuronium.
Drug to Drug IV Incompatibility: Albumin, amphotericin B cholesteryl sulphate complex, cefepime, furosemide, gatifloxacin, heparin, idarubicin, lansoprazole, omeprazole, piperacillin/tazobactam.
In syringe: Incompatible with heparin.
When admixed: Incompatible with amobarbital, chloramphenicol, chlorothiazide, dexamethasone sodium phosphate, penicillin G potassium, pentobarbital, phenobarbital, phenytoin.
Stability: Store vials at 20°C-25°C (68°F-77°F). After initial reconstitution with SWFI, D5W or NS, solutions are stable for 14 days if refrigerated. After further dilution with D5W or NS, the solution may be stored in a refrigerator for 14 days without significant loss of potency. Solutions diluted with D5NS, RL, D5RL may be stored in a refrigerator for 96 hours. Store pharmacy bulk packages at 20°C-25°C (68°F-77°F). Discard pharmacy bulk packages no later than 4 hours after initial closure puncture.

Infusion related events (anaphylactoid reactions, flushing), nephrotoxicity, ototoxicity, neutropenia, nausea, chills, fever, rashes, eosinophilia, phlebitis, decreased BP, dyspnoea, stridor, pruritus, mucosal inflammation, eosinophilia, phlebitis, severe pain at inj site, Stevens-Johnson syndrome, toxic epidermal necrolysis, bullous dermatosis (discontinue).
What happens if I miss a dose of Vancolone Injection 1 gm 1 Vial?
IV: Adults: 500 mg every 6 hr or 1 gm every 12 hr. Children: 10 mg/kg every 6 hr. Infants & Neonates: 15 mg/kg 1 dose then 10 mg/kg every 12 hr for 1st wk of life, & every 8 hr up to 1 mth of age.
